The UC Davis ROAD Program currently consists of The University of California Asthma Network (UCAN) and The COPD Case Management Program. UCAN The UC Davis Asthma Network (UCAN) is an integrated clinical service established in 1999 as a resource to primary care providers to help manage poorly controlled asthma patients identified by primary care providers or health care resource utilization(s). UCAN offers asthma patients the convenience of one-stop service with evaluations of disease control and severity, treatment recommendations in an asthma action plan, and patient education. This comprehensive multi-disciplinary chronic disease management program has markedly improved the quality of life of asthma patients while dramatically reducing emergency room visits and hospitalizations: COPD Case Management Program The COPD Case Management team at UC Davis Medical Center is aimed to improve wellness management, disease management, and acute illness management for patients admitted to our hospital with a COPD exacerbation. We are a team of 7 Registered Respiratory Care Practitioners here to provide education to COPD patients and their families about their disease, medications, breathing techniques, oxygen use, and smoking cessation. We work with Pulmonologists, Hospitalists, Primary Care Physicians, Nurses, Discharge Planning, and Pharmacy to develop individualized treatment plans and offer specialty referrals. In addition to our in hospital efforts, we offer a monthly support group for people in the Sacramento region living with COPD, called The Inspiration Club.
