The East 78th Street Houses are a row of five attached brick houses on that street in Manhattan, New York, United States. They are the remainder of an original group of 11 built in 1861, when the area was originally being developed due to the extension of rail transit into it.As a result, they are among the oldest townhouses on the Upper East Side. Some of them have been added onto, and the two easternmost were combined into a single unit. They are two stories high, with exposed basements giving them the appearance of three and a main entrance below street level. The westernmost house, 157, has had a slate-shingled mansard roof with three gabled dormer windows added. At the east end, 163 and 165 have been combined into one house, with a penthouse on the roof.

As a response to the slowly progressive decrease in cerebral blood flow, a network of dilated friable blood vessels at the base of the brain develop to feed the starved cerebral tissue. On cerebral angiography, the gold standard diagnostic test for this condition, these vessels resemble a "puff of smoke" and the name "moyamoya" is the Japanese word for this "puff of smoke" appearance. Moyamoya disease was first described in the Japanese population, however, it is now being recognized more frequently and is seen in all racial and ethnic groups across the globe. The condition can present at any age, but is typically diagnosed in children in the first decade of life or in adults (more commonly women) in their 30's. Children characteristically present with transient episodes of unilateral weakness, sometimes alternating sides between episodes. These symptoms are caused by transient ischemic attacks (TIAs) or small ischemic strokes. Adults tend to present with ischemic (lack of blood flow) strokes or brain bleeds related to rupture of the friable moyamoya vessels or brain surface collaterals that are under high demand for blood flow. Diagnosis is made using MRI and diagnostic cerebral angiography along with tests to assess cerebral blood flow including MRA NOVA and Brain SPECT imaging. The appearance of Moyamoya disease on imaging may be seen in association with other medical conditions. When the changes in the brain blood vessels are associated with another underlying medical problem, the condition is termed Moyamoya Syndrome. Medical conditions associated with Moyamoya Syndrome include sickle cell anemia, hyperthyroidism, Neurofibromatosis Type I, Down Syndrome, certain congenital heart defects, previous brain radiation therapy, and giant craniofacial hemangiomas. Treatment is directed to those patients that are symptomatic with either TIA, stroke or bleeding. There is no medical therapy for the disease, although aspirin is often recommended because it is helpful for stroke prevention. Treatment for symptomatic patients involves surgically restoring blood flow to the deprived brain through either direct bypass or indirect revascularization. During a direct bypass, a blood vessel from the scalp (superficial temporal artery) is surgically attached to a brain blood vessel (middle cerebral artery) through a small skull defect. This bypass immediately provides additional blood to the starved brain and is the technique of choice in adults if the patient's particular anatomy allows it. The indirect revascularization, or EDAS (stands for encephaloduroarteriosynangiosis) involves suturing a scalp blood vessel directly to the brain substance rather than to another blood vessel. The covering of the brain, called dura, may also be used during this operation to enhance the effectiveness of this technique. The restoration of blood flow following an EDAS procedure may take weeks to months, but ultimately provides sufficient blood flow as it matures to prevent future strokes. The EDAS technique is favored for children and is also useful for adults with the disease.
