GATE Program for Juvenile Weapon Offenders Developed by Mimi Watson Sutherland RN BSN MS CNRN Program Director Jackson Memorial Medical Center Program History The GATE Program for Juvenile Weapons Offenders was created and implemented in 1998 at Jackson Memorial Hospital by Mimi Sutherland RN BSN MS CNRN. This trauma center based experiential psycho-educational program was developed by Ms. Sutherland with the collaboration of State Attorney Katherine Fernandez Rundle in response to the increasing recidivism in both juvenile weapon use and violent trauma. At that time Ms. Sutherland was the Neurosurgical Nurse Coordinator. Ms. Sutherland retired five years ago after thirty years of service from Jackson but continues as Director of the program. After more than a decade of success the program has the continued support of the Offices of the State Attorney and the Public Defender. The program has won awards for Jackson Memorial Hospital from the National Association of Public Hospitals Safety NET Healthy Communities, Florida Department of Health and is considered by the Florida Department of Juvenile Justice to be a model program. The program has also been featured in the newspapers and on various radio and television shows. A documentary on the program has also been shown locally, nationally and internationally. Program Description The GATE Program is an interventional performance hospital based program for male non-violent, juvenile weapon offenders ages twelve to seventeen referred from the Juvenile Justice System as an alternative to long term detention. The six months long after school program is funded by the Miami Dade County Youth Crime Task Force. The program is currently housed on the fifth floor of the Highland Pavilion. under the Mental Health Hospital Center. Despite nationwide decreasing homicide and crime rates, juvenile gun and other weapons offenses continue to increase. Recidivism in the Juvenile Justice System for violent crime has also not diminished. A trauma center based program was developed for juvenile weapons offenders that provides first hand exposure to their worst fears, not of dying but of severe neurological consequences. Program Design A psycho educational approach outside the usual short-term punitive Juvenile Justice Detention Center setting is one of the basic concepts, which is based on the Health Belief Model (HBM) of change. The program involves non-violent juvenile weapon offenders in an intensive education and intervention program conducted in a medical environment that provides them with information and first-hand exposure to the consequences of violence. Education about gun behaviors and violence, while exposing young offenders to the aftermath of injury and death by weapons and violence, focuses on the traumatic consequences of pediatric victims and their families. Unhealthy behaviors and risk factors are addressed in all aspects of interpersonal violence, substance abuse, and relationships. The latter part of the program focuses on choices, decision-making skills, and attitude change. Anger management, emotional and behavioral self-control strategies, and conflict resolution skills are included to emphasize personal responsibility and accountability for ones’ actions. The skills building segment incorporates physical confrontation and communication components. In order for the juvenile offender to successfully complete this program, he must attend forty-six classes in the course totaling at least one hundred hours, prepare a presentation for the program graduation and complete any other sanctions assigned by the referring division. Graduation is a very special event for the 2 adolescents and their families. Many of them have never received a certificate of accomplishment or finished or completed anything. Many of them have never had a chance to acknowledge what they did wrong to their families and make them proud of what they have learned to change their behaviors. All the graduates have to prepare and deliver a graduation speech highlighting the most significant parts of the program for each individual. Owning up to our mistakes in public is no easier for adolescents than adults. Families have been very moved by what various instructors have said to their boys about the choices and decisions they make in their lives, and that there are many people in the community willing to support them during difficult times. Many people are willing to help economically deprived scholastic achievers, but very few are willing to help high risk underachieving adolescents especially after they had a mistake. Dissemination of Results Presentations have been provided to many entities of the Juvenile Justice System, school system, and the public health arena. The evaluation data is continuously being analyzed to assess long term results in terms of changes in truancy, school attendance, suspensions, probation violations, and grades as well as sibling and parent/caregiver relationships. The program has a high completion rate and between and ten and fifteen per cent recidivism rate for another weapons charge for graduates. There is high rate of return to the program after graduation. Over thirty graduates continued with the program and have graduated as Peer Instructors partnering with adult Instructors to help teach classes and mentor other boys. Twelve of them are still actively involved in the program some for over seven years. In fact our Program Assistant is a program graduate who now works for the program and attends college. Five other boys now teach classes as Instructors. Future of the Program The program is being funded by the Miami-Dade County Youth Crime Task Force. The scope of services has been expanded to include other weapons, a parent/sibling component, a post graduate counseling option and expansion of the Peer Instructor’s component. The South Miami/Coral Gable Elks Lodge #1676 is the current fiscal agent for the program.
